FIRST PASSPORT

• Two (2) Passport size photographs;
• Original Birth Certificate showing Parents Name/Surname (Birth certificates from Nigerian must be duly legalized in Abuja);
• Letter of consent of Parents of applicants under the age of sixteen (16);
• Copies of Parents document;
• Family Status certificate.

RE-ISSUE

• Two (2) Passport size photographs;
• Copy of Residence Permit;
• Copy of the Passport to be re-issued and the original.

REPLACEMENT

• Two (2) Passport size photographs;
• Copy of Residence Permit;
• Copy of the Passport to be replaced and the original.

LOST

• Two (2) Passport size photographs;
• Police report for lost Passport;
• Birth Certificate (if from the National Population Commission in Nigeria, must be duly legalized in Abuja);
• Evidence of gainful Employment;
• Copy of Residence Permit.
